The On-Line Registration process is as easy! The 1st step is to complete and submit the On-Line Player Application Form below. The next step is to complete the payment process. On-line credit card payments are handled through our Pay Pal account. You do not need to have a Pay Pal account to make a payment. When you have successfully completed the payment process, you will receive an e-mail from Pay Pal. If you do not receive that confirmation, the payment process was not completed, and your card was not charged.

I understand that any Fall Hoops Fest (FHF) participant who does not abide by the rules, regulations and policies established by the (FHF) is subject to dismissal without reimbursement or recourse and I hereby waive and release the (FHF) and Mesquite High School (MHS) from any and all liability for any injury or illnesses while participating in the event. I hereby authorize the directors of the (FHF) to act according to their best judgment in any emergency if I cannot be contacted. I understand that each participant is required to have their own medical and accident insurance. I also understand that the (FHF) retains the rights to use for publicity and advertisements purposes, photographs of event participants.
